I don't think Guy did love Caroline - in the special it didn't take him long to get over her and the bridesmaid said to him "if she was the love of your life you wouldn't have let her go" or something like that.

mac may be dying but he is by far the better choice! plus for those of you who have seen the alternative ending to the xmas special, mac has something to tell caroline... oh yeh hes so gonna live! mac forever ♥

Guy is very lucky that he didn't get hit on the head with a large wooden shoe. He continually tried to ruin things between Mac and Caroline.
I know it was very noble of him to give her up but please he'll be at her like a ferret up a drainpipe as soon as Mac has breathed his last.
