It's a drama; it's no more unrealistic a portrayal of its subject matter than CSI is of forensics, and that's not why most of those who watch it watch it. No-one watches in expectation of learning about how an ER in the US actually works, or if they do I'd have to wonder why.

Actually I remember reading an article a few years ago which claimed that out of all hospital drama's on telly at the time ER was the closed one to the real thing.
Having said that I would have no idea how an American ER works. The only thing I know is that in our local A&E the staff are mingers and I'm not entirely convinced that the triage nurses know much more about emergency medicine than the actors on ER.

I read that recently some of the medical accuracy on ER was slipping. It was always checked by real doctors, so that , as I understand it, the medical details were always correct. I saw interviews with real emergency doctors a while ago (can't remember where) which said that the actual injuries described, types of treatment, issues raised etc were all plausible, but what wasn't was the speed at which things happened and the number of traumas.

Of course, this is because it's a drama and was always meant to be entertainment even at it's most realistic. If you want total accuracy - well, even documentaries cheat- watch them. We know from watching things like big brother (yawn!) that seeing things as they actually happen can be really boring. It's only when you edit to get the interesting bits that it's worth watching.

So ER was always speeded up. Real ER docs would collapse with exhaustion if things really happened at that pace. That's understood. Distorting the actual medicine, is (would be?) less forgivable.

Of course also most doctors and nurses anywhere are pretty average looking, but that's Hollywood for you. We all prefer to dream a little. Yeah, no one expects their A & E to be full of Clooney's and Wyle's and Visnjic's, but a girl can hope.....
